5 minute Physio Tip – Are piriformis stretches and flexion useful to treat acute low back pain

Patients often present with acute and subacute low back pain (LBP), and pain into flexion activities eg sitting and bending.

If the patient is painful into flexion, should we provide flexion based stretches and exercises eg knees to chest, piriformis, glute or hamstring stretches? When does it make sense to provide these exercises, and when should they be avoided in the early treatment stages?

What are scenarios where flexion based movements are appropriate, and when is it completely inappropriate?
